In this presentation, we will discuss an overview of the components involved in the transition to adulthood, what to focus on based on the age of your child, and how to develop an Action Plan for your child’s transition to adulthood.
Join us as P2P discuss what health has to do with transition; who makes up the transition team and their roles, how to incorporate health transition into a medical home, healthcare financing, legal issues and timelines.
Join us as P2P discuss Medicaid programs and services, including how to qualify for Medicaid based on a child’s disability or chronic health condition (Katie Beckett Deeming Waiver). This course will include information on EPSDT, GAPP, SOURCE, and a brief overview of CCSP, ICWP and NOW/COMP waivers.
Join us as P2P provides an overview of Georgia’s Medicaid waivers, and will explore specifically the NOW/COMP waivers. You will learn about the eligibility requirements, how to apply, the services covered, and what happens once your child/young adult’s NOW/COMP waiver is funded. You will also be provided some tips and suggestions on how Parent to Parent of Georgia can help you navigate this process.
